چکیده

Breast cancer is the leading nonhematologic cause of meningeal carcinomatosis (MC), a unusual metastatic site. Its treatment generally relies on intrathecally and/or systematically delivered chemotherapy, combined with radiotherapy or surgery when needed. However, the overall survival (OS) of patients with proven MC is short, generally no longer than 6 months after the diagnosis of MC [1]. A retrospective study on 91 breast cancer MC diagnosed and treated homogeneously by a high-dose intrathecal methotrexate regimen [2] at the Institut Curie (Paris, France) from 2000 to 2007 has been recently reported [3]. This study retrieved four adverse independent prognostic factors at MC diagnosis: performance status (PS) of three to four, elevated Cyfra 21-1 marker in the cerebrospinal fluid (CSF), negative hormone receptors (HR) status and more than three previous chemotherapy lines for metastatic disease. Two prognostic scores were derived from these results, based on the number of adverse prognostic factors: the complete score included the four prognostic factors, whereas the simplified score did not include CSF Cyfra 21-1 as its dosage is not made routinely in this setting. Both prognostic scores allowed to classify patients into three groups with poor, intermediate and good prognosis (median OS of 2, 4 and 12 months, respectively). These scores required further validation. A retrospective validation of the simplified prognostic score has been conducted in patients diagnosed with breast cancer MC at the Institut Jules Bordet (Brussels, Belgium), after approval of the local ethic committee. Briefly, the electronic file of each of the 926 patients who underwent CSF cytological analysis from 01/2000 to 07/2010 were screened manually for MC diagnosis (tumor cells in CSF): 88 patients had both breast letters to the editor Annals of Oncology

Detecting meningeal carcinomatosis from breast cancer with thallium-201 SPECT.

Thallium-201 (201Tl) scintigraphy is one of the imaging methods used in the detection of various tumors including brain metastasis. We evaluated a patient with meningeal carcinomatosis from breast cancer by using 201Tl single-photon emission computed tomography (SPECT). Meningeal spread of a tumor was noted on enhanced CT. SPECT revealed tumor localization in meningeal carcinomatosis. These res...

Intracranial Leptomeningeal Carcinomatosis from Breast Cancer Detected on 18F-FDG PET.

Leptomeningeal carcinomatosis is an uncommon manifestation of non-central nervous system (CNS) metastatic disease. Diagnosis, however, has important prognostic and treatment implications. We present a case in which intracranial leptomeningeal carcinomatosis from a primary breast cancer was detected with (18)F-FDG PET/CT, despite its low sensitivity for detection of CNS metastases from non-CNS p...

Establishment and validation of circulating tumor cell-based prognostic nomograms in first-line metastatic breast cancer patients.

PURPOSE Circulating tumor cells (CTC) represent a new outcome-associated biomarker independent from known prognostic factors in metastatic breast cancer (MBC). The objective here was to develop and validate nomograms that combined baseline CTC counts and the other prognostic factors to assess the outcome of individual patients starting first-line treatment for MBC. EXPERIMENTAL DESIGN We used...

Prognostic indicators in peritoneal carcinomatosis from gastrointestinal cancer

Peritoneal carcinomatosis from gastrointestinal cancer has new treatment options for surgical management. The approach uses cytoreductive surgery which combines peritonectomy and visceral resection in an effort to remove all visible cancer within the abdomen and pelvis. Then the peritoneal cavity is flooded with chemotherapy solution in an attempt to eradicate residual disease. In order to sele...

Metronomic Capecitabine Effectively Blocks Leptomeningeal Carcinomatosis From Breast Cancer: A Case Report and Literature Review

BACKGROUND Meningeal carcinomatosis is a rare complication in breast cancer patients. At present, there are no defined guidelines for its management. The efficacy of systemic treatment seems to depend on its ability to cross the blood-brain-barrier and its interaction with tumor vasculature. Metronomic chemotherapy is a known modality of drug administration able to inhibit tumor angiogenesis. C...
